Supported Apartment Housing Program

The Supported Apartment Housing Program is a 140 bed scattered site apartment initiative designed to provide independent living options to mentally ill adults throughout Bronx County.

Our overall supported housing goal is to restore an individual's ability for independent living, socialization, and effective life management. Our supported housing provides access to affordable independent housing and support services that are based on the needs, strengths, preferences, and values of the individual. Our supported housing mission is to assist people to succeed in their housing with minimal intervention from organization staff. Consistent with our program operations, services offered by our staff reflect the cultural and ethnic diversity of our consumers.

Concrete services including, case management are offered to our supported housing consumers during the first month of residency. Thereafter and throughout the resident's stay, case management is offered as needed and in conjunction with the individual support plan. Concrete services include entitlement benefits, medical, mental health, and chemically dependent treatment; network development; community integration; and issues related to moving from the current environment.

A number of rehabilitative services are available to residents of our Supported Housing. Use of these services is strictly voluntary, delivered as needed and based upon the mutually agreed upon goals in the resident's support plan. Some services are available on-site and provided by H.O.G.A.R. staff; other services are provided off-site and available from local service providers. Access to local service providers is arranged by a case manager. Whenever possible, residents are encouraged to use generic community based resources to reduce the stigma associated with their diagnosis and to promote their integration into the community.
