Permanent Congregate Housing Program

This 47 unit permanent congregate housing program is designed to provide permanent housing for homeless single adults in the Bronx who are medically challenged and referred to H.O.G.A.R. by the NYC Human Resources Administration/HASA.

The principal objective and overall programmatic approach of the Garden Plaza Permanent Congregate Housing Program is to alleviate the difficulties associated with securing and maintaining permanent housing for people living with HIV/AIDS, and to offer and deliver needed supportive services in an effective and efficient manner whereby the these individuals can live independently and with dignity in the community for as long as possible.

In addition, H.O.G.A.R. Garden Plaza provides a range of services, including onsite case management, service plan development and review, access to substance abuse and mental health counseling, access to primary medical care, social, cultural/recreational resources and activities, educational workshops focusing on significant topics, entitlements advocacy, vocational counseling, and refinement of daily life skills such as budgeting and money management, travel training, meal preparation, housekeeping and grooming. Relocation assistance is also provided for those residents who are no longer able to function independently.

The program provides fully furnished studio apartments. The residential building offers lounge areas for residents, on site laundry, community room with kitchen, a garden, and recreational facilities.
